De Quervain's Tenosynovitis

De Quervain's Tenosynovitis is a condition which affects the tendons at the base of the thumb and wrist. Pain is normally the main complaint, which gets worse when you take your thumb outwards, gripping your hand and moving your wrist sidewards. Tucking your thumb into your hand and moving your wrist sidewards away in the opposite direction of your thumb is often the most painful movement, and a good test that the tendon is irritated.

De Quervain's Tenosynovitis is often caused by overloading the tendon through activity, but it can also be triggered if you suffer from an underlying inflammatory condition. Tenosynovitis simple means an inflammation of the tendon. A tendon attaches muscle to bone and helps transfer the energy produced by the muscle into movement.

Initially you want to support the thumb by using a splint to give the tendon some time to heal. Eventually you need to build the strength and range back to the wrist and thumb. Due to the period of relative rest the muscles and tendons will become weaker, and not addressing this will cause you likely to suffer the same outcome in the future.

As this was likely a result of an overload of activity, it is really important you understand what caused this to occur so that when you return to normal activity it doesn't happen again. Have you recently started a new hobby, perhaps you have been doing more DIY than normal, or started a new job or duty at work which is not what you normally do. Understanding the history which led to this is crucial so that you can progressively load the area to perform these activities in the future.

Tendons love load and are designed to withstand a large amount of activity, and they have the great ability of adapting to the loads we put them under. However, they tend to get irritated if the load they are put under changes too quickly to what they can adapt to, so when we started putting more load through this area of the wrist bear this in mind and progress the intensity slowly.
