This form is for subscribers of our Rehab Tracker App. Please complete this form with as much detail as possible in order for our Support Team to help resolve your issue as quickly as possible. 

Please use the email address that you used to register your App Subscription with. This is important to allow us to identify your user account. This will also be our means of communicating with you in case we cannot locate your account.
Please describe, in as much detail as possible what the issue is that you're facing.

To delete your account and all associated data, sign in at:
app.thephysiobot.com → Profile → Privacy & Data → Delete Account.
Or email support@thephysiobot.com. This removes your profile and all health/rehab data within 30 days (we retain only legally required financial records).
